Office Hours

We are open to serve you, answer questions or schedule an appointment during the following hours:

Monday: Closed
Tuesday - Friday:
7:30 AM - 5:00 PM

Appointments

We will attempt to accommodate your schedule as much as possible when making your appointments. If it’s an emergency, we will make every attempt to see you the same day.

Your scheduled appointment time has been reserved specifically for you, and we strive to stay on schedule to minimize wait time.

Since we do perform surgeries, many different circumstances may lengthen a patient’s appointment. Emergencies may also cause delays in the schedule. We appreciate your patience and cooperation with this matter.

Insurance

Our office is committed to helping you maximize your insurance benefits. Because insurance policies vary, we can only estimate your coverage in good faith but cannot guarantee coverage due to the complexities of insurance contracts. Your estimated patient portion must be paid at the time of service. As a service to our patients, we will bill insurance companies for services. If you have any questions, our courteous staff is always available to answer them.

Financial & Insurance

It is our mission to provide you with affordable, high-quality treatment that fits your budget! While we require that payment is due at the time service is rendered, we do offer a variety of payment options to meet your needs. Our office accepts payments by cash, checks, and Visa, MasterCard, American Express and Discover credit cards. If you have any questions, please call us at 602-230-7563! We will thoroughly explain your choices and work hard to accommodate your needs.

If your insurance covers treatment, you will receive the benefit of reduced personal costs. We will send you a monthly statement. Insurance policies vary, so we will review your insurance to determine the appropriate course of action.

Once treatment has started, we will file your claims. To help us with your insurance filing, please complete the insurance questions on our questionnaire.

Please remember that insurance is a benefit for you or your child. If, during the course of treatment, your benefits change, the financially responsible party is accountable for all charges.

First Visit

Consultation

Your initial appointment will consist of a consultation explaining your diagnosis and treatment options. Often, surgery can be performed on the same clay as the consultation. However, a complex medical history or treatment plan will require an evaluation and a second appointment to provide treatment on another day.

Please assist us by providing the following information at the time of your consultation:

  • Your surgical referral slip and any X-rays if applicable
  • A list of medications you are presently taking
  • If you have medical or dental insurance, bring the necessary insurance ID cards

Important: All patients under 18 years of age must be accompanied by a parent or legal guardian at the consultation visit.

A pre-operative consultation is required for patients undergoing IV anesthesia or surgery. Please have nothing to eat or drink 8 hours prior to surgery. Please remember that you will also need an adult to drive you home.

Please alert the office you have a medical condition that may be of concern prior to surgery (i.e. diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are currently taking any medication (i.e. heart medications, aspirin, anticoagulant therapy, etc.)

X-rays

If your dentist has taken X-rays, you may request that they forward them to our office. If there is not enough time, please pick them up and bring them to our office. If additional films are necessary, they can be taken at our facility.

Patient Forms

To help us with your chart, please download and print the appropriate forms, fill them out and bring them to your first appointment.
